ABSTRACT

Introducción: La cirugía laparoscópica ginecológica permite intervenir a la paciente sin tener que abrir el abdomen, pues consiste en realizar pequeñas incisiones. Actualmente se emplea en enfermedades como el cáncer o en la extracción de quistes y miomas, por lo que se puede aplicar en casi cualquier intervención de cirugía ginecológica. Objetivo: Caracterizar los procedimientos laparoscópicos del Servicio de Ginecología en el Hospital Materno-Infantil Ángel Arturo Aballí en el período comprendido entre enero del 2018 y diciembre del 2021. Métodos: Se realizó un estudio observacional, descriptivo, retrospectivo de corte longitudinal con el fin de describir la actividad asistencial laparoscópica. La muestra fue de 180 pacientes, según el criterio de selección no probabilístico de los investigadores. Resultados: La edad promedio fue 41,6 años. Según los antecedentes, predominó el tamaño del útero con 91,1 % (n = 164). Dentro de las indicaciones quirúrgicas, se encontró la paridad satisfecha en un 34,4 %, seguido del tumor de ovario (18,9 %), las neoplasias intraepiteliales cervicales (13,4 %) y el fibroma uterino sintomático. En cuanto a la cirugía mayor, se encontró la histerectomía abdominal laparoscópica en un 49,0 % (n = 78), seguido de la salpingectomía con un 38,9 %, resultados con significación desde el punto de vista estadístico (p = 0,004). En cuanto a la cirugía menor, el 11,6 % de la muestra recibió la ablación de los focos endometriales. Las complicaciones fueron escasas. Conclusiones: Los procederes laparoscópicos del Servicio de Ginecología del Hospital Materno-Infantil Ángel Arturo Aballí se consideraron adecuados de acuerdo a los indicadores quirúrgicos.


Introduction: Laparoscopic gynecological surgery allows for the patient to undergo surgery without having to open their abdomen, since it consists in making small incisions. It is currently used in diseases such as cancer or in the removal of cysts and myomas; therefore, it can be applied in almost any gynecological surgery. Objective: To characterize the laparoscopic procedures of the gynecology service at Hospital Materno-Infantil Ángel Arturo Aballí, in the period from January 2018 to December 2021. Methods: An observational, descriptive, retrospective and longitudinal study was carried out in order to describe the laparoscopic assistance activity. The sample consisted of 180 patients, according to the researchers' nonprobabilistic selection criteria. Results: The mean age was 41.6 years. Concerning antecedents, uterine size predominated, with 91.1% (n = 164). Among surgical indications, satisfied parity was found in 34.4%, followed by ovarian tumor (18.9%), cervical intraepithelial neoplasms (13.4 %) and symptomatic uterine fibroid. With respect to major surgery, laparoscopic abdominal hysterectomy was found in 49.0 % (n = 78), followed by salpingectomy, with 38.9%; such results are statistically significant (p = 0.004). Regarding minor surgery, 11.6% of the sample received ablation of endometrial focuses. Complications were rare. Conclusions: The laparoscopic procedures of the gynecology service at Hospital Materno-Infantil Ángel Arturo Aballí are considered adequate according to the surgical indicators.

Resumen OBJETIVO: Determinar las características clínicas y los desenlaces asociados con las diferentes vías de acceso elegidas para la histerectomía. MATERIALES Y MÉTODOS: Estudio retrospectivo, descriptivo y transversal consistente en el análisis de la información asentada en los expedientes de pacientes atendidas en el servicio de Ginecología del Hospital Civil de Guadalajara entre el 1 de marzo del 2019 y el 28 de febrero del 2020. En todas las pacientes se estableció la asociación entre la vía de acceso y las complicaciones. RESULTADOS: Se revisaron 790 expedientes de los que 413 reunieron los criterios de inclusión. Se practicaron 262 histerectomías abdominales, 107 por vía vaginal y 44 mediante laparoscopia. La frecuencia de complicaciones fue de 9.2%, el 54% se registró durante el procedimiento quirúrgico. La frecuencia de conversión fue de un caso para histerectomía vaginal (0.9%) y de otro para la laparoscópica (2.2%). La complicación más frecuente fue el sangrado transquirúrgico seguida del reingreso por infección del sitio quirúrgico. La cirugía vaginal fue la que menos complicaciones porcentuales registró; sin embargo, la lesión vesical fue la más común y la única en la que sucedió una defunción, secundaria a choque hipovolémico. La asociación entre la vía de acceso abdominal y las complicaciones muestra que las pacientes con histerectomía abdominal tuvieron tres veces más probabilidades de alguna complicación en comparación con el resto de los grupos. CONCLUSIONES: El riesgo de complicaciones es mayor en la histerectomía abdominal, sobre todo en pacientes con peso uterino ≥ 500 g e IMC ≥ 30. Es necesario reforzar las competencias para incrementar la frecuencia de la histerectomía vaginal. Cada vez es más necesario estar a la vanguardia en la aplicación de técnicas de mínima invasión por su ventaja de menor frecuencia de complicaciones.


Abstract OBJECTIVE: To determine the clinical characteristics and outcomes associated with the different access routes chosen for hysterectomy. MATERIALS AND METHODS: Retrospective, descriptive and cross-sectional study consisting of the analysis of the information recorded in the records of patients attended in the Gynecology service of the Civil Hospital of Guadalajara between March 1, 2019 and February 28, 2020. The association between the access route and complications was established in all patients. RESULTS: 790 files were reviewed of which 413 met the inclusion criteria. A total of 262 abdominal hysterectomies were performed, 107 vaginally and 44 laparoscopically. The frequency of complications was 9.2%, 54% of which occurred during the surgical procedure. The frequency of conversion was one case for vaginal hysterectomy (0.9%) and another for laparoscopic hysterectomy (2.2%). The most frequent complication was trans-surgical bleeding followed by readmission for surgical site infection. Vaginal surgery had the lowest percentage of complications; however, bladder injury was the most common and the only one in which a death occurred, secondary to hypovolemic shock. The association between abdominal access route and complications shows that patients with abdominal hysterectomy were three times more likely to have some complication compared to the rest of the groups. CONCLUSIONS: The risk of complications is higher in abdominal hysterectomy, especially in patients with uterine weight ≥ 500 g and BMI ≥ 30. Skills need to be strengthened to increase the frequency of vaginal hysterectomy. It is increasingly necessary to be at the forefront in the application of minimally invasive techniques because of their advantage of lower frequency of complications.

ABSTRACT

Abstract Objective To summarize the available evidence of TAP Block in efficacy in laparoscopic or robotic hysterectomy. Data Sources We searched databases and gray literature for randomized controlled trials in which transversus abdominis plane (TAP) block was compared with placebo or with no treatment in patients who underwent laparoscopic or robot-assisted hysterectomy. Method of Study Selection Two researchers independently evaluated the eligibility of the selected articles. Tabulation, Integration, and Results Seven studies were selected, involving 518 patients. Early postoperative pain showed a difference in the mean mean difference (MD): - 1.17 (95% confidence interval [CI]: - 1.87-0.46) in pain scale scores (I2=68%), which was statistically significant in favor of using TAP block, but without clinical relevance; late postoperative pain: DM 0.001 (95%CI: - 0.43-0.44; I2=69%); opioid requirement: DM 0.36 (95%CI: - 0.94-1.68; I2=80%); and incidence of nausea and vomiting with a difference of 95%CI=- 0.11 (- 0.215-0.006) in favor of TAP. Conclusion With moderate strength of evidence, due to the high heterogeneity and imbalance in baseline characteristics among studies, the results indicate that TAP block should not be considered as a clinically relevant analgesic technique to improve postoperative pain in laparoscopic or robotic hysterectomy, despite statistical significance in early postoperative pain scale scores. Resumo Objetivo Resumir as evidências disponíveis sobre a eficácia do bloqueio TAP em histerectomia laparoscópica ou robótica. Fontes de Dados Pesquisamos bancos de dados e literatura cinza por ensaios clínicos randomizados nos quais o bloqueio do plano transverso do abdome (TAP na sigla em inglês) foi comparado com placebo ou com nenhum tratamento em pacientes que foram submetidos a histerectomia laparoscópica ou assistida por robô. Métodos de Seleção de Estudos Dois pesquisadores avaliaram independentemente a elegibilidade dos artigos selecionados. Tabulação, Integração e Resultados Sete estudos foram selecionados envolvendo 518 pacientes. A dor pós-operatória precoce apresentou diferença nasmédias (DM) de: -1 17 (intervalo de confiança [IC] de 95%: - 1 87-0 46) nos escores da escala de dor (I2=68%) o que foi estatisticamente significativo a favor do uso do bloqueio TAP mas sem relevância clínica; dor pós-operatória tardia: DM 0001 (IC95%: - 043-044; I2=69%); necessidade de opioides: DM0 36 (95%CI: - 0 94-168; I2=80%); e incidência de náuseas e vômitos com diferença de 95% CI=- 011 (- 0215-0006) a favor do TAP. Conclusão Com moderada força de evidência devido à alta heterogeneidade e ao desequilíbrio nas características basais entre os estudos os resultados indicam que o bloqueio do TAP não deve ser considerado como uma técnica analgésica clinicamente relevante para melhorar a dor pós-operatória em histerectomia laparoscópica ou robótica apesar da significância estatística nas pontuações da escala de dor pósoperatória inicial. ABSTRACT

Background: Laparoscopic surgery is associated with more favorable clinical outcome than conventional open surgery. This might be related to the magnitude of tissue trauma and tissue stress response. This study compares the intensity of tissue injury by assessing plasma C-reactive protein (CRP), lactic dehydrogenase (LDH), and cancer antigen 125 (CA 125) in patients undergoing laparoscopic hysterectomy compared to abdominal hysterectomy.Methods: This study was conducted at Ain Shams University Maternity Hospital from May 2018 to February 2020. 74 women candidate for hysterectomy were recruited and randomized into two groups: group A included 37 cases who underwent abdominal hysterectomy, and group B included 37 cases that underwent laparoscopic hysterectomy.Results: CRP, LDH and CA125 significantly increased post-operatively in both groups. Postoperative mean serum CRP, LDH and CA125 were significantly higher in the laparotomy group (10.84±2.47, 262.21±76.77, and 13.41±2.6 respectively) compared to laparoscopy group (7.92±2.25, 148.53±43.56, 11.79±2.77 respectively) (p<0.05).Conclusions: Surgery is a significant cause of tissue injury that can be assessed by specific markers. We suggest that laparoscopic surgery causes less tissue damage as assessed by lower postoperative CRP and LDH values, which might account for the earlier recovery and reduced hospital stay in laparoscopic surgery.

RESUMEN Introducción: La histerectomía constituye la segunda causa más frecuente de cirugía mayor en la mujer. Tres tipos de histerectomías son usadas en la actualidad: la histerectomía vaginal, la histerectomía abdominal y la histerectomía mínimamente invasiva. Esta última variedad se comenzó a realizar en Matanzas a finales de la pasada década. Objetivo: Evaluar las características clínicas epidemiológicas de las pacientes histerectomizadas por cirugía video laparoscópica. Métodos: Se realizó un estudio descriptivo y prospectivo. El universo lo conformaron todas las pacientes en las que se implementó esta técnica quirúrgica y se tomó como muestra al propio universo, 96 pacientes. Resultados: Predominó la edad comprendida entre 40 y 49 años con 44 pacientes; la raza negra con 51 pacientes; diagnóstico de miomatosis uterina con 87 casos; tiempo quirúrgico de 61-90 min con 57 pacientes y estadía hospitalaria de hasta 24 h en 93 de las pacientes. Se presentaron 3 complicaciones en total y el número de casos convertidos fue 0. Conclusiones: La histerectomía videolaparoscópica es implementada de forma segura y eficaz. Se realizó predominantemente en mujeres mayores de 40 años, con diagnóstico de miomatosis uterina. El tiempo quirúrgico promedio es mayor que el reportado en la literatura. La estadía hospitalaria es de 24 horas como promedio y se reincorporaron a sus actividades habituales rápidamente(AU)


ABSTRACT Introduction: Hysterectomy is the second most frequent type of major surgery among women. Three types of hysterectomy are currently used: vaginal hysterectomy, abdominal hysterectomy, and minimally invasive hysterectomy. This last variety began to be performed in Matanzas at the end of the last decade. Objective: To evaluate the clinical-epidemiological characteristics of patients hysterectomized. Methods: A descriptive and prospective study was carried out. The population was made up of all the patients operated on by this surgical technique. The whole population itself was taken as a sample (96 patients). Results: There was predominance of the age group 40-49 years (44 patients), the black race (51 patients), diagnosis of uterine myomatosis (87 cases), surgical time of 61-90 min (57 patients), and hospital stay of up to 24 hours (93 patients). There were three complications and the number of converted cases was 0. Conclusions: Videolaparoscopic hysterectomy is implemented safely and effectively. It was performed predominantly in women over 40 years of age and diagnosed with uterine myomatosis. Average surgical time is greater than that reported in the scientific literature. Hospital stay is 24 hours as an average. The patients returned to their usual activities quickly(AU)

ABSTRACT

Background: Diseases of the female genital tract are commonly encountered in clinical practice. Hysterectomy is the definite treatment for many of the pelvic pathologies. The present study is aimed at analysis of age and types of hysterectomy and evaluation of histopathological reports of the hysterectomy specimens.Methods: A total of 476 consecutive hysterectomy were studied over a period of two years from January 2016 to December 2017.Results: The peak age group of hysterectomy was 41-50 years with 221 (46.43%) cases. The commonest type of hysterectomy was abdominal. The most common endometrial pathology was atrophic endometrium, seen in 131 (27.52%) cases. In myometrium, the most common pathology was leiomyoma in 179 (37.61%) cases. Among cervical lesions, chronic cervicitis was the most common finding, seen in 274 (57.56%) cases.Conclusions: The experience with various types of hysterectomies at our institution has been reviewed. A wide spectrum of lesions were observed when histopathology reports of the hysterectomy specimens were analysed.

Background: Hysterectomy is the second most common operation performed by the gynecologists, next only to caesarean section. Objective of the study was to compare fall in blood haemoglobin level, duration of operation, intra- and post-op complications between non-descent vaginal hysterectomy and total laparoscopic hysterectomy and establish the better method for hysterectomy in non-descent uterus.Methods: A retrospective comparative study of 90 hysterectomies was done from a period of May 2018 - April 2019 at GMERS Medical College and Hospital Sola, with 45 cases in group of non-descent vaginal hysterectomy (NDVH) and 45 in group of total laparoscopic hysterectomy (TLH). Demographic characteristics, co-morbid conditions, indications for surgery, operative time, intra- operative blood loss, post-operative analgesia requirements, post-operative hospital stay and post-operative complications were compared between both groups. Those patients having malignancy as diagnosed by Pap smear or by D and C were excluded from the studyResults: The most common age in both groups was 41-50 years. Adenomyotic uterus was the most common indication for surgery in both groups. The mean operative time in NDVH group was 45 min while it was 80 min in TLH group. p<0.001 suggested significant difference when operative time were compared between both groups. Both groups were similar in post-operative analgesia requirement and post-operative hospital stay. Post-operative complications were similar in both groups.Conclusions: In which way to approach the uterus shall depend upon skill of the surgeon, size and pathological nature of uterus, technology available in the hospital and preference of patient as well as surgeon.

ABSTRACT

Background: Hysterectomy is one of the most common gynecological surgeries performed around the world. This study was done to review the data of all patients who underwent TLH. With this study a uniform method of performing TLH by a single surgeon was assessed.Methods: A 5 years retrospective study was performed at Sri Dharmasthala Manjunatheshwara College of Medical Sciences and Hospital Dharwad, Karnataka India. Demographic data, pre-operative findings, indication for surgery, intra-operative and post-operative complications, duration of surgery were recorded and analyzed.Results: A total of 118 women were included in the study. Mean age of the patients wasMost common indication for the surgery was leiomyoma. Mean operating time was 162.18 minutes. Mean weight of the post-operative specimen was 208.45 grams. Intra-operative urinary bladder injury was seen in 1 patient. Conversion to laparotomy was needed in 2 patients.Conclusions: With improving experience TLH can be performed safely without complications. As number of surgeries done increases duration of TLH reduces. TLH can be effectively used to avoid laparotomy.

ABSTRACT

Background: This study is aimed to review indications, demographic data of patients, clinical outcomes and safety of total laparoscopic hysterectomy.Methods: This is a prospective observational study of total 150 patients who underwent total laparoscopic hysterectomy (TLH) from 1st June 2017 to 30th November 2018 in GMERS Civil Hospital Sola.Results: ~45% patients were between 40-50 years age group; 60% patients had 2 or more deliveries; commonest indication was symptomatic adenomyosis ; uterine size in ~57% of patients were up to 6 weeks; duration of surgery in ~91% of patients <120 minutes; intraoperative blood loss in all cases <200ml; no intra-operative and postoperative complications were encountered.Conclusions: TLH is safe procedure with minimal blood loss, minimal postoperative pain and discomfort and shorter duration of hospital stay when performed via expert hands.

Background: Abdominal hysterectomy is the most frequented way of hysterectomy in the world; today we have a lot of techniques for hysterectomy. Total Laparoscopic Hysterectomy (TLH) versus Total Abdominal Hysterectomy (TAH) needs experience an assessment of the learning curve. Objective of the study was to determine the feasibility and safety of TLH and TAH.Methods: Total 100 women were taken for study. Operating time, estimated blood loss, operating complication and length of stay in hospital were noted for each patient. The success rates of TLH were more compared to TAH. The operating time estimated blood loss, conversion to laparotomy was directly proportional to size of uterus.Results: There were no statistically significant differences between the two groups regarding age, body mass index (BMI), specimen weight, pre-operative hemoglobin (Hb) value and rates of the complications. The mean post-operative Hb value was significantly higher in group TLH than group TAH (11.3±0.7 gr/dl versus 10.6±1.6, p = 0.03). The mean time of operation was significantly longer in TLH than group TAH (105.4±22.9 minutes versus 74±18, p<0.001). The mean duration of hospital stay was statistically shorter in TLH compared to the TAH (2.47±0.5 days versus 4.86±1.1, p<0.001).Conclusions: Advantage of TLH over TAH are less blood loss, fewer wound infection and fever, smaller incisions, with less pain, shorter hospitalization time, speedier recovery.

ABSTRACT

Background: Laparoscopic hysterectomy has become a widely accepted technique worldwide, since its introduction by Reich in 1989. The aim of the study was to compare the surgical results and intra- operative and post-operative complications between total laparoscopic hysterectomy (TLH) and total abdominal hysterectomy (TAH).Methods: Authors conducted a retrospective study at SMGS hospital, GMC Jammu between October 2016 to September 2018, 126 patients who underwent TLH and 126 patients who underwent TAH, were included.Results: Authors observed that there was no significant difference between the two groups in respect to patient’s age, parity and indication of surgery. Duration of surgery was found to be longer in TLH than TAH. The length of hospital stay was less in TLH than TAH. Hb drop was more in TAH group. 25 patients of TAH were given post op blood transfusion but only 9 patients of TLH were given post op blood transfusion. The time to ambulation in patients of TLH was much shorter than TAH. Wound infection was more common in TAH as compared to TLH, 5 patients had wound infection and in 4 patients resuturing was done for wound dehiscence. In 8 patients laparoscopy was converted to laparotomy because of adhesions, vascular injury and in one case because of bowel injury.Conclusions: This study showed that TLH can be safely performed by the experienced surgeon as an alternative to abdominal hysterectomy. It offers several benefits over TAH such as smaller incision, earlier ambulation, shorter hospital stay, faster recovery time and does not increase more serious complications than TAH.

Resumen OBJETIVO: Comparar la frecuencia de dehiscencia de la cúpula vaginal en pacientes tratadas con y sin estrógenos equinos conjugados por vía vaginal antes de la histerectomía total laparoscópica. MATERIALES Y MÉTODOS: Ensayo clínico controlado, comparativo, ciego simple, efectuado en pacientes atendidas entre enero de 2013 y agosto de 2016 en el Hospital Civil de Culiacán con indicación de histerectomía total laparoscópica por enfermedad benigna. Criterios de inclusión: haber recibido durante tres semanas previas al procedimiento estrógenos equinos conjugados por vía vaginal (grupo de estudio) o crema lubricante vaginal como placebo (grupo control). Criterios de exclusión: pacientes con diabetes mellitus tratadas con esteroides y antecedente de un evento tromboembólico reciente. Se evaluó la frecuencia de dehiscencia de la cúpula vaginal a los 7 y 30 días posteriores a la cirugía. Se utilizó la prueba t de Student para comparar medias y la χ2 para la comparación de proporciones, con cálculo de riesgo relativo para evaluar el riesgo de dehiscencia de la cúpula vaginal. RESULTADOS: Se estudiaron 236 pacientes con edad promedio, en ambos grupos, de 47.1 y 47.7 años, respectivamente (p > .05). Los antecedentes ginecoobstétricos y las indicaciones para histerectomía fueron similares en ambos grupos (p = .340). La incidencia total de dehiscencia de la cúpula vaginal fue de 4.6% (n = 11/236) con frecuencia de 6.8% (n = 8/118) en las pacientes del grupo tratado con lubricante vaginal y de 2.5% (n = 3/118) en el grupo tratado con estrógenos equinos conjugados por vía vaginal, sin diferencias entre ambos grupos (p = .123; RR=.359; IC95%: .093-1.387). CONCLUSIONES: El riesgo de dehiscencia de la cúpula vaginal fue similar entre el grupo que recibió lubricante y el de estrógenos tópicos vaginales, pero con una tendencia menor en la frecuencia de dehiscencia de la cúpula vaginal en el grupo tratado con estrógenos tópicos.


Abstract OBJECTIVE: To compare the frequency of vaginal cuff dehiscence in patients managed with and without conjugated equine estrogens vaginally prior to total laparoscopic hysterectomy (TLH). MATERIALS AND METHODS: In a single-blind controlled clinical trial, 236 patients with indications for laparoscopic total hysterectomy for benign pathology at the Hospital Civil de Culiacán and who agreed to participate in the study were randomly assigned to receive vaginally conjugated equine estrogens (study group) or vaginal lubricating cream as a placebo (control group) for 3 weeks prior to the procedure. Patients with diabetes mellitus, treated with steroids and history of a recent thromboembolic event were excluded. The frequency of vaginal cuff dehiscence was evaluated at 7 and 30 days after surgery. The student's t-test was used to compare means and the Chi-square test was used to compare proportions, with relative risk (RR) calculation to evaluate the risk of vaginal cuff dehiscence. RESULTS: We studied 236 patients with average age, in both groups, 47.1 and 47.7 years, respectively (p > .05). The mean age of the patients was similar between the groups (47.1 vs 47.7 years, p>.05). The gynecological-obstetric history and indications for hysterectomy were similar between both groups (p = .340). The incidence of vaginal cuff dehiscence was 2.5% (n = 3) in the group treated with conjugated equine estrogens vaginally and 6.8% (n = 8) in the patients of thse group treated with vaginal lubricant (p= .123; RM= .359; IC95%: .093-1.387). CONCLUSIONS: The risk of presenting vaginal cuff dehiscence was similar between the group of lubricant and topical vaginal estrogens, but with a lower tendency in the frequency of vaginal cuff dehiscence in the group of topical estrogens.

ABSTRACT

OBJECTIVE@#To compare the efficacy and safety of postoperative analgesia with low-dose sufentanil combined with transversus abdominis plane (TAP) block and with sufentanil alone in promoting patients'recovery following laparoscopic hysterectomy.@*METHODS@#Sixty patients undergoing laparoscopic hysterectomy in our hospital between September, 2016 and August, 2017 were randomly allocated into two equal groups. In group A, the patients were given postoperative analgesia with 1 μg/kg sufentanil, 9.96 mg tropisetronmesylate, and 200 mg flurbiprofen axetil (diluted with 0.9% NaCl solution to 100 mL, pumped at the rate of 2 mL/h) combined with TAP block; in group B, the patients received similar postoperative analgesia but at a higher dose of sufentanil (2 μg/kg) without TAP block. Visual analogue scale (VAS) was used to evaluate pain at 15 min and at 4, 8, 12, 24 and 48 h postoperatively, and the first off-bed time, the length of postoperative hospital stay and the incidence of postoperative nausea and vomiting (PONV) were recorded in all the patients.@*RESULTS@#Compared with those in group B, the patients in group A had significantly lower VAS scores at 15 min, 4 h, 8 h, and 12 h postoperatively ( < 0.01) with also statistically shorter first off-bed time and postoperative hospital stay ( < 0.01). Two (6.7%) patients in group A had mild PONV, and 6 (20.0%) in group B had PONV (including 4 with mild and 2 with moderate PONV).@*CONCLUSIONS@#Lowdose sufentanil combined with TAP block is effective for postoperative analgesia after laparoscopic hysterectomy and helps to reduce the incidence of PONV and shorten the first off-bed time and postoperative hospital stay to promote the recovery of the patients.

ABSTRACT

Introducción: Uno de los factores que han influido para lograr una mayor aceptación de la histerectomía totalmente laparoscópica es la introducción de dispositivos que permiten mejorar la movilización del útero durante la cirugía. Objetivo: Evaluar el manipulador uterino KD de producción nacional en la histerectomía totalmente laparoscópica. Método: Estudio prospectivo longitudinal en una serie inicial en humanos. El univero estuvo conformado por las pacientes con patologías uterinas tributarias de tratamiento quirúrgico. La muestra la constituyeron 50 pacientes que cumplieron los criterios de inclusión. Las variables en estudio fueron: realización de la histerectomía, efectos adversos del sistema rígido en el manipulador diseñado, del sistema neumo oclusor, indicación de la operación, peso del útero, sangramiento intraoperatorio, índice de masa corporal, tiempo quirúrgico, morbilidad intraoperatoria y posoperatoria. El análisis estadístico fue realizado por una especialista en Bioestadísticas, mediante el programa estadístico IBM-SPSS versión 21 para Windows en español. Como medidas de resumen, las frecuencias absolutas, frecuencias relativas tales como el porcentaje, la media y el rango, así como la asociación de Spearman. Resultados: Solo existió un efecto adverso con el manipulador uterino: la perforación de un útero de 55 g. El mioma uterino fue la principal indicación de la histerectomía, el tiempo quirúrgico; así como el sangrado intraoperatorio, estuvieron acordes con otros resultados. Existió una correlación entre peso del útero, sangrado y tiempo quirúrgico. Conclusiones: La histerectomía totalmente laparoscópica con el uso del manipulador uterino diseñado se realizó en la totalidad de las pacientes con resultados aceptables(AU)


Introduction: One of the factors that have influenced for achieving greater acceptance of the fully laparoscopic hysterectomy is the introduction of devices that allow to improve the mobilization of the uterus during surgery. Objective: To assess the uterine manipulator KD of domestic production, in the totally laparoscopic hysterectomy. Method: Longitudinal prospective study in an initial series in humans. The total sample was made up by patients with uterine pathologies and who were candidates for surgical treatment. The sample consisted of 50 patients who met the inclusion criteria. The variables under study were hysterectomy, adverse effects of the rigid system on the designed manipulator, pneumo-occlusion system, indication of the operation, uterine weight, intraoperative bleeding, body mass index, surgical time, intraoperative and postoperative morbidity. The statistical analysis was carried out by a specialist in Biostatistics, using the statistical program IBM-SPSS version 21 for Windows in Spanish. As summary measures, we used absolute frequencies, relative frequencies such as the percentage, the mean and the range, as well as the Spearman association. Results: There was only an adverse effect with the uterine manipulator, which was the perforation of a 55-gr uterus. The uterine myoma was the main indication for hysterectomy; the surgical time, as well as intraoperative bleeding, were consistent with other results. There was a correlation between uterine weight, bleeding and surgical time. Conclusions: The fully laparoscopic hysterectomy with the use of the designed uterine manipulator was performed in all of the patients and with acceptable results(AU)

Background: Total laparoscopic hysterectomy (TLH) is now emerging as a safe procedure even in patients suitable for vaginal hysterectomy (VH) due to its advantages like better visualisation, less post-operative pain and shorter hospital stay. This study was conducted to compare between laparoscopic hysterectomy and vaginal hysterectomy in females with benign disorders. Materials and methods: This was retrospective study conducted in Department of Obstetrics and Gynaecology, Laxmi Narasimha Hospital, Warangal. The patients who were indicated for vaginal hysterectomy or total laparoscopic hysterectomy were included. This study was conducted during the period of July 2015 to October 2016. Forty eight patients were included in the TLH group who underwent total laparoscopic hysterectomy and forty three patients were included in the VH group who underwent vaginal hysterectomy. Results: Operation time was higher in TLH group which was 112.56 hours when compared to VH group which was 101.42 hours (p<0.001). Hospital stay was more in VH group which was 5.77 days when compared to TLH group which was 4.05 days (p<0.001). Analgesia dosage was higher in VH group which was 1.64 when compared to that in TLH group which was 1.28 (p<0.001). The rate of complications in both the groups was found to be non-significant statistically. One woman who was undergoing VH had bladder injury which was recognised intraoperatively and treated and another one had vault hematoma in post-operative period which was also managed. In one case of VH, conversion to laparotomy was done due to difficulty but all cases were posted for TLH. Conclusion: Compared to Vaginal hysterectomy, total laparoscopic hysterectomy was more advantageous as it showed lesser hospital stay and lesser analgesia dose.

Resumen OBJETIVO: Demostrar si se cumple lo publicado en referencia a las curvas de aprendizaje en histerectomía laparoscópica, para un mismo equipo quirúrgico, en variables como el porcentaje de complicaciones, conversiones a laparotomía, tiempo quirúrgico, pérdida de hemoglobina, días de hospitalización, etcétera. MATERIALES Y MÉTODOS: Estudio analítico, observacional de cohortes, prospectivo y de intervención efectuado de julio de 2014 a octubre de 2017 en el Departamento de Ginecología del Hospital General Santa María del Puerto, Cádiz, España. Criterio de inclusión: pacientes con histerectomía total o supracervical laparoscópica. RESULTADOS: Se analizaron 45 procedimientos divididos en 3 cohortes de 15 pacientes cada una de acuerdo con el orden temporal de realización. Así, el grupo 1 fue el de las primeras 15 histerectomías efectuadas, el grupo 2 de la 16 a la 30, y el grupo 3 de la 31 a la 45; es decir, las últimas 15 llevadas a cabo. El porcentaje de complicaciones y conversión a laparotomía fue de 13.3% en el grupo 1, de 6.7% en el grupo 2 y de 0% en el 3 (p = 0.343). El tiempo quirúrgico medio en el grupo 1 fue de 164 minutos, en el 2 de 101 minutos y en el 3 de 90 minutos (p = 0.001). Entre los diferentes grupos se registraron mejoras progresivas estadísticamente significativas en la pérdida de hemoglobina o la estancia hospitalaria. CONCLUSIONES: Con base en nuestros resultados la curva de aprendizaje es de 45 intervenciones, suficientes para practicar con estándares de seguridad este tipo de cirugías.


Abstract OBJECTIVE: To observe the learning curve in a surgical team of laparoscopic hysterectomy. Determine the number of surgeries needed to achieve a surgical time 90 minutes average, a percentage of total complications less than 10% and a conversion to laparotomy rate to less than 5%. MATERIAL AND METHODS: For this we have analyzed data collected prospectively, in 45 patients undergoing total laparoscopic hysterectomy or laparoscopic supracervical hysterectomy, carried out by the same surgical team, and divided into 3 cohorts of 15 patients by temporal order of preparation; made July 2014 to October 2017, in the Department of Gynecology of the Hospital General Santa Maria del Puerto. RESULTS: We analized 45 procedures divided into 3 cohorts of 15 patients each according to the temporal order of performance. Thus, group 1 was that of the first 15 hysterectomies performed, group 2 from 16 to 30, and group 3 from 31 to 45; that is, the last 15 carried out. The complications and conversion rate to laparotomy was 13.3% in group 1; 6.7% in group 2, and 0% in group 3 (p = 0.343). The mean surgical time in group 1 was 164 minutes, in 2 of 101 minutes and in 3 of 90 minutes (p = 0.001). Among the different groups there were statistically significant progressive improvements in the loss of hemoglobin or hospital stay. CONCLUSIONS: Our results indicate that a learning curve of 45 interventions is sufficient to deal with this type of surgery with safety standards.

Introducción: es necesario sopesar los beneficios y riesgos de la implementación de la histerectomía totalmente laparoscópica frente a la abdominal. Objetivos: describir el comportamiento de la histerectomía totalmente laparoscópica en el Hospital General Docente Iván Portuondo de San Antonio de los Baños. Métodos: estudio observacional descriptivo en una serie de casos, en el Hospital General Docente Iván Portuondo de San Antonio de los Baños, desde enero de 2013 hasta diciembre de 2015. La muestra quedó constituida por 36 pacientes con diagnósticos que necesitaron se les realizara histerectomía por vía abdominal. Los datos se recopilaron y procesaron mediante el uso del paquete estadístico SPSS versión 21.0 para Windows. Se realizaron los gráficos, cuadros y análisis correspondientes a las variables estudiadas, causa de la operación, peso estimado del útero medido en gramos, complicaciones intraoperatorias y posoperatorias, tiempo quirúrgico, cantidad de sangrado, estadía hospitalaria y tiempo de reinserción a la vida cotidiana. Las variables cuantitativas y cualitativas se expresaron en sus respectivas medidas de resumen: por ciento y el establecimiento de intervalos de confianza para las variables cuantitativas, así como la posible asociación entre ellas. Resultados: el mioma uterino fue la principal indicación de la cirugía, en la medida que se avanzó en la curva de aprendizaje, se disminuyó en el tiempo quirúrgico y la cantidad de sangrado. El sangramiento de la arteria uterina fue la principal complicación, que provocó conversión en tres de las pacientes operadas. Del total de pacientes, 91,6 por ciento de nuestras pacientes tuvieron una corta estadía hospitalaria y 80,5 por ciento una rápida reinserción a la vida cotidiana. Estos resultados estuvieron relacionados con la introducción de un nuevo proceder(AU)


Introduction: It is necessary to weigh the benefits and risks of the implementation of entire laparoscopic hysterectomy versus abdominal hysterectomy. Objectives: Describe the behavior of the entire laparoscopic hysterectomy in Iván Portuondo General Teaching Hospital in San Antonio de los Baños. Methods: A descriptive observational study was conducted in a series of cases, at Iván Portuondo General Teaching Hospital in San Antonio de los Baños, from January 2013 to December 2015. Thirty-six (36) patients formed the sample. Their diagnoses confirmed the need of performing abdominal hysterectomy. The data was collected and processed by using the statistical package SPSS version 21.0 for Windows. The graphs, charts and analyzes corresponding to the variables studied, cause of the operation, estimated weight of the uterus measured in grams, intraoperative and postoperative complications, surgical time, amount of bleeding, hospital stay and time of reintegration into daily life were performed. The quantitative and qualitative variables were expressed in their respective summary measures: percent and the establishment of confidence intervals for the quantitative variables, as well as the possible association between them. Results: Uterine myoma was the main indication for surgery, as the learning curve progressed, the surgical time and the amount of bleeding decreased. Bleeding of the uterine artery was the main complication, which caused conversion in three of the operated patients. 91.6 percent of our patients had a short hospital stay and 80.5 percent a rapid reintegration into daily life. These results were related to the introduction of a new procedure(AU)

Background: Hysterectomy is a frequently performed operation for the benign conditions of uterus, but the route of hysterectomy is always a matter of discussion .Laparoscopic hysterectomy has been criticised many a time regarding its complication and duration of surgery. But availability of newer machines and techniques has popularised it recently. Most of the studies regarding this have been done by the single surgeon with expert hand .This study has been conducted in a university teaching hospital where there are many surgeons with different expertise along with the beginners. Methods: It is a retrospective observational study conducted in IMS and SUM hospital Bhubaneswar which is an university teaching hospital.286 Total laparoscopic hysterectomy patients between July 2014 to June 2015 were analysed regarding the demography, indication of operation ,duration of operation, intra operative blood loss and perioperative complication along with duration of hospital stay. Result: Major indication of surgery was fibroid uterus accounting for 40.20% followed by AUB 28.32%. Mean time of surgery in TLH is 2.34±0.67hrs and average blood loss is 150.9±58.8ml. 4.54% cases had major intra operative complications; minor post operative complications were in 14.68% cases. The conversion to laparotomy rate was 1.39% and 0.68% cases had urinary tract injury. Mean duration of hospital stay was 2.58± 1.98days. Conclusion: TLH is a safe procedure with minimal blood loss and shorter duration of hospital stay with surgical expertise .we can never ignore the machine behind the man.

ABSTRACT

Objective To explore the value of ureteroscopy in the diagnosis and treatment of ureteral injury caused by laparoscopic hysterectomy . Methods Suspect ureteral injury occurred in 23 cases out of 578 laparoscopic hysterectomy from January 2010 to December 2015 in our hospital.Among them, 12 cases were testified by ureteroscopy .A double-J catheter was placed in for 3 months. Results All the 12 cases were followed up for 3-26 months (average, 19 months).The double-J catheter was removed at 3 months after surgery .The continuity of the affected side urinary tract was restored in 10 cases and hydronephrosis was not found by B ultrasound .Two cases were found ureteral obstruction and were given ureterovesical reimplantation at 3 months after surgery . Conclusions The ureteroscopy should be carried out as early as possible for patients with suspect ureteral injury after laparoscopic hysterectomy .Double-J catheter placement can avoid parts of patients converting to open surgery .

ABSTRACT

Perioperative stroke can lead to mortality or serious disability and usually occurs in patients undergoing cardiac, vascular, or neurologic surgery; it is rare in gynecological surgery. We report the case of a patient who suffered life-threatening cerebral infarction after elective laparoscopic hysterectomy. During the surgery, the patient was placed in the Trendelenburg position. On postoperative day one, the patient was diagnosed with right hemisphere cerebral infarction; brain computed tomographic angiography showed proximal right internal carotid artery occlusion. Decompressive craniectomy was performed to resolve brain swelling, but the patient died 10 days later.
